Social Services at Lincolnshire Healthcare Center and Rehabilitation:
The social services team at Lincolnshire Healthcare Center is here to be of assistance to both our patients and their families during their stay with us. We can help with the transition from your previous living situation to an enjoyable stay in our center. We find ways to meet the social and emotional needs of our residents, and make sure they are comfortable during their stay here. While the resident is the main focus of the social worker’s efforts, the social worker will also be available to work with the patient’s family as needed. 
A representative from social services will be a part of the interdisciplinary team for resident care planning. They are available to counsel residents and their families if necessary. They will also be connecting residents with appropriate community resources and will serve as a liaison between the resident and other agencies if needed. They will advocate for residents’ rights, and work to ensure that each resident’s social and emotional needs are being met.
Social services is also responsible for seeing that residents have opportunities for social contact and interaction of various kinds. They support the maximum level of independence for each resident during their stay.
